Mission: To provide services that maintain, integrate and enhance the lives of seniors in the community

Programs: Milestones adult day health centers (cedar rapids and marion) provide adult day services to health-challenged individuals who may require support, supervision, and structure to remain as independent as possible. These programs served 420 consumers and their families during fy15, with approximately 89% considered low income. The milestones programs are the only adult day health centers in our community. They provide a service that meets an otherwise unmet need. Without these programs, individuals would be forced to move to nursing homes or other care facilities at a much higher cost. Since most of the individuals receiving services are considered low income, the adult day health centers reduce the government's financial burden.

the case management program offers assistance to older adults in meeting health, social, recreational, housing, or personal needs. During fy15, the case management program served 2,337 seniors, allowing them to remain in the community and avoid a higher level of care. More than 90% of those receiving services are considered low income.

custodial care: residents of the home pay a monthly fee for custodial care based on what they can afford. The average fee charged is less than half of the actual cost of their care which is subsidized by investment income. The full fee is still under the total cost of care.
