Mission: Our Mission is to meet everyone’s basic needs and to increase the quality of life of adults with disabilities, adults with autism and the elderly. To support people’s independence, we empower them so they can participate in daily decision-making and in directing their services.
Target demographics: Adults with disabilities, Adults with Autism and the Elderly
Direct beneficiaries per year: We are currently servicing close to 3000 participants throughout Pennsylvania
Geographic areas served: Pennsylvania
Programs: Community Health Choices, Adult Autism Waiver and ACT 150/OBRA waivers. Providing Case Management Services to individuals so that they can remain in their homes with the supports needed to remain in the community
