Causes: Military & Veterans Organizations, Veterans

Mission: Code of support foundation provides essential and critical one-on-one assistance to those struggling service members, veterans and their families who have the most complex needs. We are dedicated to leveraging the nation's full spectrum of resources to ensure all members of our military, veterans and their families receive the support services they need and through their collective sacrifice have earned.

Programs: Case Coordination - the foundation expanded our case coordination program, and added veteran peer navigation services. We added both additional staff and volunteers, and more than tripled the number of service members, veterans and family members we served to over 325 people. Our team of trained case coordinators and veteran peer navigators are dedicated to helping military and veteran families in crisis. We work to identify, prioritize and fulfill the often multiple and complex needs of these families, by partnering with a network of organizations across the country to ensure their successful transition into civilian life. Within the past two years, we have seen tremendous expansion in the demand for our case coordination services and we are rapidly increasing our efforts to meet that demand.
PATRIOTlink - Created to support the efforts of our case coordinators, when we recognized that the amount of time spent on identifying the multiple resources required to support our holistic approach, was not scalable. For every one need identified, a coordinator had to contact an average of eight organizations to find the appropriate resource. We knew we needed a technology platform to support the efforts of our case coordinators and we knew that if it helped our team be more efficient and effective, it would help service providers across the country. PATRIOTlink is the nation’s first cloud-based veteran resource navigation platform designed by service providers, for service providers.
Education & Engagement - the foundation hosted two significant education and engagement events during 2014.Spirit of 45 day, held at the World War II memorial in Washington, dc, brought together over 400 civilian and veteran Americans. Participants worked shoulder to shoulder to build a living wall of honor, the nation's first "live" (the wall was made up of over 1,000 flowers) memorial to honor post 9/11 veterans. The wall was subsequently moved to the women's memorial, at Arlington national cemetery where it was re-dedicated at another event on September 11th, to all female service members who died in support of post 9/11 operations.
