Mission: Ucp works to advance the independence, productivity, and full citizenship of people experiencing physical or invisible barriers.

Programs: Home and housing program: the home and housing program works with clients and caseworkers to provide home modification solutions for those with disabilities. Ucp home and housing staff currently provide bid-coordinating, project management and contractor services to caseworkers for each project. The home and housing program also includes housing access coordination services. Housing access coordination staff work closely with clients to research and arrange appropriate housing solutions. Once the solution is agreed upon by clients and case managers, ucp staff coordinate the move, transition to the new location and conduct follow up contacts to insure a good fit.

assistive technology program exists to provide needed technology, devices, advice and support for people who often have limited resources to acquire devices/services they need. Since assistive technology is often complicated and specialized to each specific need, it is an expensive and time-consuming process to fit an individual with the best possible solution. This process can be exasperating for a person dealing with daily challenges of a disability. As a result, those with disabilities often settle for less than ideal solutions, higher costs and low-functioning results. The assistive technology program brings expertise and support alongside individuals and families when researching, testing and purchasing technology/equipment. Equipment purchased through ucp services can be used for communication, employment, employment searching, physical needs, education or home automation. Ucp works to create vendor relationships so technology can be taken for a "test run" to insure the best fit for each individual need. Ucp also collects available tech into a library of testable devices for a spectrum of adaptations that can be accessed quickly and loaned for accurate testing results. Having an advocate throughout the process relives stress, eases the timeline and allows each person to make the better purchase decisions. As a result, not only do individuals receive better outcomes but also state waivered funds are not wasted on incorrect technology purchases or underused/unused technology.

community navigator program: ucp community navigators work directly with clients and caseworkers to find the best client-centered solutions. Community navigators provide waiver education, participate in the human services council, support local services groups and are the experts in building bridges to community resources. The community navigator program provides outreach to counties and service providers, leading to new partnerships and contract opportunities, whilemaintaining our current contracts. Increasing the number of county partners and contracts allows ucpcm to serve more clients while providing the counties with cost-effective client enrollment assistance, assessments, services, and advocacy.

inclusive community program: the inclusive community program builds bridges, allowing those with disabilities access to activities and events they would not normally have. The program works to create a calendar of activities that provide social, physical and mental benefits for all ages, levels of disability and their families/care givers.
