My info didn't line up and it took me alot of trial and error to for it to print on the lines correctly. I had to adjust the percentage in paper setup in case anyone is having the same issue. Arrived sealed in a clear bag without any manual or info.

The forms and envelopes were good quality. I am taking a star off because they had NO template available to use these forms - I had to create my own workaround which took a long time with a lot of trial error to get things lined up..Note that I use Quickbooks for accounting but do not use (pay for) the payroll option which then does not allow Quickbooks to print directly onto these forms. I only have a few of these 1099's to create once a year so it is not worthwhile to pay for payroll option for the entire year.This company sells other labels that they DO provide free templates for but, unfortunately, not for this product..
